HIP's Depression Disease Management Program
Depression is the most common behavioral health condition in this country, affecting more than twenty million Americans. While highly effective treatments for depression are available, practitioners often report that treating depressed patients is difficult and sometimes frustrating. Depressed patients often tend not to comply with their prescribed treatment regimen and prematurely stop taking their medications.
HIP has created a depression disease management program whose main objective is to improve the process and the outcome of depression care by developing a partnership with the treating clinician. The program is free and members may disenroll at any time.
The Depression Disease Management program serves members diagnosed with Major Depression Disorder who have had either frequent ER visits (at least two in a year) or inpatient care for depression. Those patients who do not meet this criteria may not need such closely coordinated care, and we can help you find an alternative treatment that is appropriate for them.
The Depression Disease Management program provides the following services:
- Educating members about depression and available treatments.
- Information about adjunct services.
- Help in making appointments.
- Coordination of care.
- Monitoring of treatment adherence.
- Monitoring of treatment progress using the Patient Health Questioniare-9 (PHQ-9).
- Progress reports to the referring practitioner.
