HIP is pleased to offer this web site dentist directory as a reference. Please remember that all information within this dentist directory is subject to change. We recommend that members confirm provider participation when scheduling an appointment, or at some point prior to visiting the provider. We also recommend verifying your eligibility by entering your social security number, in the space provided, on the provider search page.
Additionally, this listing represents all participating dentists in the dental benefit plan for those members enrolled through a group or organization (such as an employer, labor union, association or welfare fund that acts as a remitting agent and sends HIP the premium for the member's coverage), those subscribers enrolled through a HIP/HMO direct payment contract effective prior to 01/01/96; and all Medicare members.
This directory does not apply to Child Health Plus members, Medicaid members, HIP/HMO Direct subscriber, HIP/Choice Plus Direct subscriber or healthy New York members.
HIP arranges for the delivery of oral health care services in accordance with and subject to the terms of your certificate of coverage entered into between you or on your behalf, and HIP. HIP does not directly provide these services. Rather, these services are provided by Independent Contractors. The dental providers listed in this directory that deliver oral health care services are not the employees or agents of HIP. HIP shall not be liable for any negligent act or omission by any of the providers listed in this directory, or any of their employees or agents, who may, from time to time provide dental services to you. HIP expressly disclaims any agency relationship, actual or implied, with any health care provider. Any decisions made by HIP concerning appropriateness of setting, whether any service or supply is dentally necessary, pursuant to this Agreement shall be deemed to be made solely for the purposes of determining whether benefits are due under the Agreement between you and HIP, and not for the purposes of recommending any dental treatment or non-treatment. HIP does not exercise any control or direction over the medical/dental judgement or clinical decisions of any dental provider listed in this directory and does not interfere with the dentist patient relationship between you and any dental provider. It is important to know when you enroll in HIP, services are provided through HIP's delivery system, as described in your HIP Certificate of Coverage, but the continued participation of any one dentist cannot be guaranteed.
