How to become an appointed FMO with Emblem Health:
FMOs seeking appointment must submit the following documents:
- A copy of your current New York State corporate broker license is under Section 2104 of the insurance law, issued under section (a) of the insurance law that stipulates Accident & Health accreditation.
- A completed W-9 form
- Errors and Omission Insurance
- GHI/HIP Selling Agent Appointment Form
- A completed EmblemHealth FMO contract
How to gain appointment for recruited agents/brokers through an FMO:
FMOs seeking appointment for their agents/brokers must submit the following documents:
- A copy of their New York State Accident & Health license
- Errors and Omission Insurance
- Assignment of Commission form
- GHI/HIP Selling Agent Appointment Form
- A completed EmblemHealth FMO Contract
Training/Testing Requirement:
In accordance with CMS guidance on Broker/Agent Training and Testing, section 42, CFR 422-2274 (b) and 423.2274(b)—CMS 4138 – IFC:
Medicare Advantage Organizations (MAO) and Part D sponsors must ensure, starting with sales and marketing for plan year 2009, that annually brokers and agents selling Medicare products are trained on Medicare rules and regulations and on plan details specific to the plan products being sold by the brokers and agents selling. MAOs and Part D sponsors must also ensure that brokers and agents selling Medicare products are tested annually on their knowledge of Medicare rules and regulations, as well as, on the plan specific details of the plan products being sold. In order to sell Medicare products, a broker or agent should receive a passing score of at least 85% on the test. Tests may be in the form of a written test or computerized.
For plan year 2010, EmblemHealth will conduct an electronic CMS developed training on Medicare Regulations. Agents/brokers whom complete and pass this on-line course will be credited for the compliance portion of the training. The product training/testing will be held at EmblemHealth and requires reservation. FMOs must contact the Broker Unit* for training/testing dates.
