Personal Health Passport
To keep your heart healthy, it is important to "Know your numbers." That's why HIP has customized the Personal Health Passport for its members.
You can use your Personal Health Passport to record important numbers like your blood pressure and cholesterol results.
Important Information:
- Blood pressure: If your blood pressure is below 120/80, it is normal. If it is higher, consult your doctor.
- Cholesterol: If your total cholesterol is above 200, or if your LDL is above 100, consult your doctor. Smokers and people with diabetes, high blood pressure, heart disease and others at high risk should keep levels even lower. The lower your LDL, the lower your risk for heart disease and stroke.
What To Do:
- Take your Personal Health Passport with you every time you visit your doctor.
- Make an appointment to see your doctor this year and ask him or her to help you fill out your Personal Health Passport.
