Behavioral Health Group Program for Members with Diabetes

Program Description:
  • 5-session, structured group conducted by a behavioral health specialist. After the 5 weekly sessions, monthly sessions continue.
  • All members with Diabetes are welcome to join the group program. Loved ones are welcome to attend the groups with the member.
  • There is no cost for the program.
  • The program is offered throughout the HIP system.
Program Focus:
  • The goal of the program is to help members with diabetes to identify obstacles to proper self-care and to learn techniques for making lifestyle changes in order overcome obstacles and improve the care of their diabetes.
Program Offers:
  • Tips/techniques for making and maintaining positive behavioral changes.
  • Strategies for identifying barriers to proper self-care.
  • Problem-solving skills.
  • Goal setting skills.
  • Mood management techniques.
  • Relaxation training.
  • Information on connections between diabetes and mood.
  • Social support and encouragement.
  • Access to HIP booklets/information on diabetes.
