Behavioral Health Group Program for Members with Diabetes
Program Description:- 5-session, structured group conducted by a behavioral health specialist. After the 5 weekly sessions, monthly sessions continue.
- All members with Diabetes are welcome to join the group program. Loved ones are welcome to attend the groups with the member.
- There is no cost for the program.
- The program is offered throughout the HIP system.
- The goal of the program is to help members with diabetes to identify obstacles to proper self-care and to learn techniques for making lifestyle changes in order overcome obstacles and improve the care of their diabetes.
- Tips/techniques for making and maintaining positive behavioral changes.
- Strategies for identifying barriers to proper self-care.
- Problem-solving skills.
- Goal setting skills.
- Mood management techniques.
- Relaxation training.
- Information on connections between diabetes and mood.
- Social support and encouragement.
- Access to HIP booklets/information on diabetes.
- The program is offered at sites throughout the five boroughs and Long Island.
