Need to change your name, address or correct your date of birth?

Did you know that HIP cannot permanently change Medicare, Family Health Plus, or Medicaid member's personal information in our files?

To change personal information such as your name, address or correct your date of birth, Medicare member's must contact their local Social Security office* and Medicaid/Family Health Plus member's must contact their Local Department of Social Services (LDSS).

State, Federal or NYCTA/TWU employees please contact your Benefits Administrator or Human Resource Department.

These offices will then give HIP your new information and we will update our records. Follow this process and you will be sure to get all important information mailed to you about your HIP benefits and service enhancements!

Keep in mind that when you call HIP Customer Service with a request for information, our Customer Service Advocates will confirm your address. If your address differs from the one in our files, we will take your current address so that you will receive the requested information. However, this will not permanently change the address in your member file. To make a permanent change, you must contact your local Social Security Office, Local Department of Social Service or your Benefits Administrator or Human Resource Department.

*Medicare members: if you are moving within your county, please ask your Social Security Office for an award letter at the time of change and forward a copy of this letter to HIP Medicare Enrollment Department, 55 Water Street, New York, NY 10016 (Medicare does not inform HIP of these changes).
